Match the rider and recurring schedule
Following the transportation provider's privacy process, capture rider identity, callback contact, pickup address, dialysis facility, appointment time, treatment days, and whether the request is one-time or recurring. Confirm the applicable time zone and date for every exception.
Record mobility and access facts
Ask only approved operational questions about wheelchair or ambulatory status, steps, elevator access, escort needs, service animal, and required vehicle features. Do not decide medical fitness for transport or infer assistance needs from a diagnosis.
Check administrative prerequisites
Record payer or authorization reference, broker or facility contact, and documents reported as submitted. Mark each as provided, missing, or pending verification. Eligibility, coverage, driver assignment, and vehicle availability must remain unconfirmed until the responsible system or person approves them.
Read back both sides of the trip
Confirm pickup location, appointment facility, requested pickup window, return-call process, and contact number. For a round trip, state how the return is triggered. Use the provider's escalation instructions for same-day disruptions; never imply that recording the request guarantees the ride.
