Headline finding

This completed review of eight authoritative sources found that a failed telephone challenge means only that the specified authentication path did not establish its required result. It is not evidence that the caller is fraudulent. NIST separates identity proofing, authentication, and federation into 3 distinct assurance dimensions, each with levels suited to different risks 1. Its authentication guidance says authentication assurance level 2 requires proof of possession and control of 2 distinct authentication factors and treats use of the public switched telephone network for out-of-band authentication as restricted 2. The FCC's spoofing guidance further shows why displayed caller ID cannot by itself settle identity 4.

All 8 documents were reviewed. Four directly addressed authentication or fraud resistance, 2 addressed governance and privacy, and 2 addressed recovery or effective access. No private call cohort, challenge event, account decision, false-rejection rate, or fraud outcome was measured. The review's counts describe source coverage only.

Research question

What can authoritative guidance establish about failures in caller authentication, and which conclusions require local call-level evidence? For a virtual assistant call center, the relevant boundary is between routing a request, authenticating control of an approved factor, authorizing a requested action, and disclosing protected information. These are not interchangeable.

Methodology

The corpus was frozen on August 17, 2026. Eight public sources qualified because they addressed digital identity, authenticator controls, multifactor authentication, spoofing, identity recovery, cyber governance, privacy, or effective communication. The primary unit of analysis was one source. The extraction form used six codes: factor requirements, channel restriction, anti-spoofing caution, fallback or recovery, disclosure minimization, and access or accommodation.

Counts use 8 as the denominator. Explicit statements were coded; silence was not treated as opposition. Vendor identity claims and behavioral voice-inference material were excluded. The review did not inspect account records, challenge answers, call audio, or caller identity. It therefore cannot estimate how often authorized or unauthorized callers failed.

Evidence interpretation

Four of 8 sources directly support authentication and anti-fraud controls. The other 4 establish governance, privacy, recovery, and communication constraints. The 2-factor AAL2 finding is tied to NIST's assurance model. It does not mean every phone inquiry requires AAL2, nor that asking two knowledge questions satisfies it. The restricted PSTN status does not mean every use is forbidden.

The synthesis supports at least nine mutually exclusive local result classes: primary path complete, approved fallback complete, comparison mismatch, factor unavailable, caller declined, interaction barrier, technical failure, abandoned, and evidence unavailable. A local denominator would be all challenge sequences triggered under one policy version. Since no such sample was reviewed here, this article reports no completion or failure proportion.

Decision boundaries

A human security and service owner must select assurance and authorization requirements for each action. A virtual assistant may collect only approved factors, enforce retry limits, and route unresolved cases. It should not invent questions, reveal stored answers, infer identity from accent or confidence, or bypass an unmet requirement.

Failure can justify withholding a protected action under policy. It cannot justify labeling a caller as malicious. Sensitive healthcare, financial, legal, safety, complaint, and account recovery decisions require the authorized human path and any applicable privacy, accessibility, and legal review.

Limitations

The corpus applies digital identity guidance to a telephone context, but not every call is a digital account transaction. Organizational obligations and threat models differ. The sources do not provide a shared call dataset or a universal challenge design. A document-level count is not evidence about callers.

Successful authentication can still involve compromised factors, and an authorized caller can fail because records are stale, audio is poor, a relay is involved, or a required factor is unavailable. No false acceptance, false rejection, caller harm, or fraud denominator was observed. This review cannot determine legal identity or intent.

Reproducibility

Retrieve the eight numbered sources, freeze publication versions and access dates, then code one document per row for the six criteria. Preserve exact section references and calculate code totals over a denominator of 8. A second reviewer should independently verify the NIST assurance and PSTN statements because they carry the main technical boundary.

A later call review should publish the transaction class, policy version, trigger query, factor dictionary, result codebook, fallback version, exclusion and unavailable counts, and reviewer agreement. Original event codes should remain unchanged after adjudication. These instructions specify what future empirical evidence would require; they do not imply that a private cohort was studied here.
