Which transfer-summary fields survive a live call handoff?
A blinded audit design for testing required-field completeness before and after transfer without equating documentation with service success.
Finding and scope
The Joint Commission and AHRQ publish handoff resources in healthcare, and NIST guidance supports controlled information transfer. Those materials show why structured handoffs matter in their settings; they do not prove which fields improve a commercial call-center transfer. This article proposes a local documentation audit and reports no performance finding.
Evidence base
AHRQ TeamSTEPPS includes structured communication tools. The Joint Commission's Sentinel Event Alert on hand-off communications discusses risks of inadequate handoffs in healthcare. NIST SP 800-53 Rev. 5 includes controls for information flow and integrity. These sources inform the method but do not validate a universal call summary.
Proposed method
Define a short required-field rubric for one queue, such as caller name, callback number, reason, completed verification step, stated urgency, promised action, and receiving owner. Sample eligible transfers across shifts. Blinded reviewers score the pre-transfer record and the receiving record, with a second reviewer adjudicating ambiguous fields.
Prespecify complete preservation of all applicable fields as the primary outcome. Report each field separately, not-applicable decisions, reviewer agreement, transfer type, hold duration, and whether the caller had to repeat information. Keep sensitive content out of the analytic extract wherever possible.
Inference limits and decision boundary
Complete fields do not prove correctness, understanding, resolution, or safety. Longer summaries may score well while burdening agents and callers. Differences between teams can reflect case mix and system design. The assistant can populate approved fields, but the receiving owner remains responsible for verifying consequential facts.
Limitations
Reviewers see only recorded artifacts and may miss spoken context. Required fields can favor routine cases, while privacy rules may appropriately suppress some details. Monitoring can change behavior, and one queue's rubric may not transfer to another industry or risk level.
